Thank you for your willingness to assist me in the usability testing of the early prototype of my advising course. To begin, please complete the consent and recording release and then complete the find a time to meet survey so I can schedule your usability test on my calendar.

Introduction and Usability Test
We will be doing these items together as we conduct the usability test. I will be your moderator if you can meet with me synchronously over Zoom. If synchronous meetings are impossible, I have also prepared the usability test to be unmoderated with remote observations through a recorded interface. As a final means of ensuring the usability of the Advising 101 course, self-paced tasks are presented in the Instructions/Activities section of this outline.
Exit Ticket and Feedback Form
These are two follow-up surveys with a slightly different focus. The exit ticket lets you immediately reflect on your usability experience, while the feedback form is anonymous to allow your honest thoughts, feelings, and input. I will send out the feedback/feed-forward form after all participants have completed their usability testing to preserve the identity of those completing it.
